Investigations
Since 1992,
we have successfully investigated hundreds of industrial and non-industrial
Insurance claims. We let our clients determine how and when they
are updated during an investigation, if at all. Most of our clients
simply prefer to give us our assignment and the next time we speak
is when we turn in our completed assignment. We furnish our clients
with clear/concise reports, and professional quality film on DVD
format, or whatever format the client prefers. Finished reports
are sent to our clients either via the Internet, hand delivered,
and/or sent via the United States Postal Service, or all of the
above.
Our sub-rosa
reports contain still color photographs, inserted in the final
written report. These pictures are taken from the surveillance
video, depicting highlights and/or identification of the subject.
This allows the client to visually follow along, while reading
the report. We also print out good identification photos of the
subject.
A typical
case starts with our own "In-house" preliminary investigation.
We only obtain prior W/C records, if needed, or if asked. We then
go over the information furnished to us, to determine what, if
anything, we need to add. If we are doing "sub-rosa",
our field work consists of activity checks to determine the proper
method of surveillance. Once this preliminary check is completed,
we proceed with the actual surveillance/investigation. We adhere
to state laws and have strict guidelines regarding personal privacy.
We also adhere to the NAIC's Model Act regarding personal privacy
as it relates to insurance investigations.
We utilize
state-of-the-art surveillance systems for acquiring both indoor
and outdoor film and our sub-rosa success rate is over 98 percent.
All surveillance's are done with two investigators utilizing our
own radio network. We also utilize a satellite communications
network that enables investigators to be in constant contact with
clients, if needed, via radiophone and/or the Internet. We do
not carry any "personal case data" into the field and
all our "case data" on subjects is encrypted in our
office at the Department of Defense level. Case information is
only in the office of the President, Douglas Hansen, where it
stays. Therefore, it is simply impossible for our firm to put
any of our clients at risk pertaining to the loss of sensitive
information.
Douglas Hansen,
our President and CEO, is a graduate and valedictorian of the
United States Drug Enforcement Administration's Academy in Washington,
DC, and has been conducting in-depth undercover investigations
for over 34 years. Our film success rate, is the direct result
of skills sharpened and fine tuned, while conducting actual surveillance's
with expert agents from the Federal Bureau of Investigation and
the United States Drug Enforcement Administration. Only working
with experts like these, can anyone ever hope to be as successful
as our firm is. It takes a highly skilled investigator to catch
today's clever insurance fraud perpetrators.

AOE/COE
means "arising out of employment, or during the "course
of employment" and requires investigators to gather all of
the factual information and statements ASAP. Incident scene
photos are also very important, so WE GET PICTURES & THE FACTS
ASAP.
Sub Rosa:
Is Latin for "under the rose" where meetings were held
in secrecy Centuries ago. The term is now used by insurance firms
referring to video surveillance of claimants. When doing video
surveillance, two investigators are used at no additional cost
to the client. We furnish our clients with "still photographs"
in our reports and DVD Disks. (VHS can still be made upon request
at no charge)

Gate Insurance Services operates under the corporation of Fog
City Investigations, a well known Private Investigation Firm located
in the San Francisco Bay Area. GGIS is a full service Insurance
Investigation Firm. We have "Investigators" located
throughout California, Oregon, Florida, and New York. GGIS is
comprised of highly trained and experienced insurance investigators,
specializing in AOE/COE, Sub-Rosa,statement taking and Subrogation
matters. GGIS handles many different insurance cases, with Workers
Compensation claims as our Specialty. We can handle simple statement
taking of injured workers and witnesses after an incident, to
taking pictures of the actual site and conducting an in-depth
AOE/COE investigation. Many times surveillance or (Sub-Rosa) has
to be employed to reveal the truth, or to determine the actual
extent of injury, if any.
